If you have not previously established your online access, register for online services to get started.

Yes. Please note that we comply with industry best practices including federal healthcare
and financial privacy regulations to ensure your personal health information is protected.

Sign up online to setup automatic payments using a bank account or credit card. You can also complete an automatic payment authorization form
and mail the form along with your statement stub and a canceled check to the payment address listed on your statement stub.

To stop your automatic payment, please notify us at least three days in advance of the next payment.

The account holder can contact our Customer Care Team at 866.625.8532 during these times to authorize someone else to access the account:

Monday to Thursday: 7:00 AM – 7:00 PM (CST)

Friday: 7:00 AM – 7:00 PM (CST)

The account holder can authorize someone else to access the account online, or by completing and submitting a Third Party Authorization Form by mail.
